Important Dates for 2024-2025 Registration

INCARNATION SCHOOL’S ANNUAL OPEN HOUSE will be held on Sunday, February 25, from 12:00 p.m. to 1:30 p.m.  All interested families are invited to attend whether you are new to the school or just interested in information.  Come tour the school and visit with our teachers and staff.

REGISTRATION for all NEW families will begin online November 15 or in person during our Open House on Sunday, February 25, from 12:00 p.m. to 1:30 p.m.  Required at registration are your child’s original birth certificate (with County seal), social security card, and certificates for any sacraments they have received (Baptism, First Communion, etc.) Also required is a $200 per family registration fee. The Health Examination Form included in the Admission Application packet is due by June 1.


SCREENING for other grades may also be required. Families who have submitted applications for new students in grades one through eight will be contacted to schedule a placement test.

The items listed below must be submitted before your Admission Application can be processed. Copies will be made of original documents at the time of registration.

  • Application for Admission
  • Birth Certificate (issued by County)
  • Social Security card
  • Sacrament Certificates (Baptism, Reconciliation, First Communion – if applicable)
  • Recommendation form – Students applying for Grades 1-8, completed online
  • School Health Form – to be completed by your physician
  • Registration Fee ($200 per family – non-refundable)
